Palliative Care Advanced Practice Registered Nurse - PRN

POSITION SUMMARY The Palliative Care Advanced Practice Registered Nurse Practitioner with prescriptive authority, will work in conjunction with the Palliative Care Team to provide services to patients in the hospital and community based setting.
The nurse practitioner will facilitate goals of care discussions and transitions to hospice along with making recommendations for symptom management.
QUALIFICATIONSEducation:
Master's Degree in Nursing Licensure:
Advanced Practice Registered Nurse licensed in Arkansas, Registered Nurse licensed in Arkansas, prescriptive authority r, including DEA certificate, required.
Experience:
Minimum of two years of RN or APRN palliative care experience preferred.
Minimum of two years administrative experience preferred Additional Requirements:
Valid Arkansas driver's license, personal automobile insurance, personal cell phone, current TB skin test, ability to travel extensively and work flexible hours.
PRIMARY RESPONSIBILITIES Utilizes approved clinical guidelines and evidence based protocols when providing care in the hospital and community based setting.
Performs comprehensive and focused evaluation of patients.
Provides services related to health maintenance and promotion, palliative care and end of life care.
Recommends laboratory and radiology tests as appropriate, in collaboration with the palliative care physician.
Collaborates with home health nurse, hospital nurse, attending and palliative care physicians and others as needed to discuss identified patient needs and to order medication adjustments, treatments, and other care.
Performs other therapeutic or corrective measures as indicated, including urgent care as necessary.
Consults with Chief Medical Officer or designee as needed, informs primary physician of services provided and collaborates with other physicians as needed; provides written reports to primary physician upon request.
Prepares and maintains accurate patient records, charts and documents to support sound medical practice.
Provides training for staff, medical residents and students as needed.
Perform other duties as requested.
Adheres to the practice of confidentially regarding patients, families, staff and the organization.
Maintains current knowledge and skills as an Advanced Practice Registered Nurse Practitioner.
Utilizes and models excellent customer service skills at all times; seeks opportunities to assist patients, families and co-workers; demonstrates teamwork and cooperation.
Accepts direction and follows instructions from supervisor; seeks additional information as needed; works with minimal supervision.

Maintain and improve the quality measures Work with Health Information Management to document on reportable palliative care measures.
Effectively communicates and collaborates with home health and nursing facilities to develop patient centered plan of care.

Knowledge of computer software applications.
