RN Case Manager
GENERAL
Summary:
Responsible and accountable for case by case clinical, quality, and financial outcomes for patients in their caseload.
Activities include concurrent clinical management, discharge planning, social work and utilization review.
REQUIREMENTS:
Minimum of 2 years clinical experience.
Ability to manage conflict and negotiate during difficult situations.
Self motivated.
Must be able to set priorities at all times.
Must be very detail oriented and follow-up on everything.
Must be able to work closely with physicians, nurses and other ancillary departments.
Abides by Quality Expectations with Service Oriented attitude.
Must demonstrate professional level of conduct, appearance and attendance.
